What shifting blocks at the top actually mean
The top course of a block retaining wall is the least loaded part of the whole structure. Nothing sits on top of it holding it down, so it's the first thing to move when the wall gets pushed.
That push comes from behind, almost always. Soil and water pile pressure against the back of the wall, and if there's nowhere for that force to go, the top blocks slide out toward you.
Here's the good part: because the top moves first, you're getting a warning. Most of the time this shows up long before anything dramatic happens lower in the wall.

The most common reasons the top drifts out
Water is the usual villain. When drainage gravel and the perforated pipe behind the wall silt up, rainwater has nowhere to escape. Saturated soil gets heavy, and that weight pushes the wall.
The second big one is simple: the cap blocks were glued down, and the glue quit. Exterior construction adhesive breaks down under UV and freeze-thaw over five to ten years. Once the bond fails, the caps wander.
In cold regions, frost heave does its own damage. Water in poorly draining backfill freezes, expands, and shoves the top blocks out a little each winter. The movement doesn't fully reverse, so it stacks up.
On taller walls, missing geogrid is a serious possibility. That's the buried mesh that ties the soil to the blocks. Skip it on a 5-foot wall (plenty of DIY builders do) and the top will slowly walk out under load.
Base problems and new loads round out the list. They're less common for top-only movement, but worth ruling out if the obvious suspects don't fit.
