The second big cause is the automatic fill valve creeping. It's supposed to hold cold pressure around 12 psi. When it sticks open, it keeps feeding until pressure climbs past the relief setpoint.

Third, the valve itself just wears out. Years of tiny weeps etch the sealing seat until it can't close tight. Once that happens, no amount of pressure fixing will make it reseal. It needs replacing.

How to diagnose it yourself

Start cold. Look at the pressure gauge before the boiler has fired in a while. You want to see around 12 psi. If it reads 20, 25, or higher when cold, your fill valve is overfeeding.

Now let the boiler run through a full heat cycle and watch the gauge. A healthy system might climb a few psi. If it rockets up toward 30, your expansion tank isn't absorbing the expansion.

Check temperature too. If the water is climbing near 210 degrees F, stop. That's an overheating problem, not a plumbing one, and it needs a service call.

Test the expansion tank. Rap your knuckles on it top to bottom. Healthy tanks sound hollow up top (air) and dull at the bottom (water). A waterlogged tank sounds solid and feels heavy all over.

On a diaphragm tank, put a tire gauge on the air valve. With the system depressurized on the water side, the air charge should match your target fill pressure, usually 12 psi. Low or zero means the bladder's shot.

If pressure is normal cold and hot, and the tank checks out, but the valve still weeps, you've found your answer. The valve is worn and needs to go.

Snap a photo of both gauges cold, then again after a full heat cycle. Comparing the two numbers side by side makes the diagnosis obvious.
